Understanding Medicare IRMAA
What is IRMAA?
IRMAA stands for Income-Related Monthly Adjustment Amount. It is a surcharge added to your Medicare Part B and Part D premiums if your income exceeds certain thresholds.
2-Year Look-Back
Your 2026 IRMAA is based on your 2024 tax return (MAGI). This means income from two years ago determines what you pay today.
The "Cliff" Effect
IRMAA uses income cliffs, not gradual increases. Going just $1 over a threshold can increase your premiums by over $1,000 per year.
VA Disability Shield
VA disability compensation is excluded from MAGI under federal law (26 USC 104). Medicare cannot see it. Enter your taxable income only — not your disability pay.
